Disability Benefits Questionnaire (DBQ)

DBQs are official VA examination templates used to document the nature and severity of a specific condition. A well-prepared DBQ can make the difference between an approved and a denied claim. Because thorough documentation matters, our providers currently offer DBQs via telehealth for the following conditions:

  • Migraine Headaches
  • Sinusitis
  • Sleep Apnea

Why Claims Get Denied

Don't Let Paperwork Cost You

Every year, thousands of deserving Veterans are denied the benefits they've earned — not because their conditions aren't real, but because the paperwork doesn't tell the full story. Understanding the most common reasons for denial is the first step toward making sure it doesn't happen to you.

Missing Medical Evidence

Without documented proof of your condition, the VA has no foundation to approve your claim.

Incomplete Documentation

Gaps in your records can leave your claim vulnerable, even when your condition is legitimate.

Lack of Service Connection

The VA requires a clear, professional link between your current condition and your military service. Without it, your claim has little ground to stand on.

Insufficient Medical Rationale

A diagnosis alone is rarely enough. The VA expects detailed, evidence-based reasoning that meets their specific standards.

Don't let a paperwork problem stand between you and the benefits you've earned.

Preparing For Your Intake

What To Have Ready

Your intake interview can be completed your way — by email, phone, or virtual appointment. Don't worry if you don't have everything — we'll work through it together.

Part 1

Your Military Service

We'll start with the foundation of your claim. Be ready to share your service branch, MOS, and the dates of service reflected on your DD214. If you served as a Guardsman with additional service periods, bring those records as well. Deployment history — including locations — can also play a significant role in establishing your claim.

Part 2

How Your Condition Began

This section helps us establish the origin of your injury or illness. Think about whether there was a specific event — a fall, accident, or physical strain — or whether your condition developed gradually through repeated wear and tear during service. Any supporting documentation from that time, such as sick-call slips, military physicals, or Lines of Duty, can be valuable. Personal statements, buddy statements, or letters from a spouse or family member can also strengthen your case.

Part 3

Your Current Medical Condition

To build an accurate timeline, we'll need to understand where things stand today. Be prepared to share your current diagnosis, who made it, and approximately when. We'll also want to know what steps you've taken to manage your condition over the years — from early adjustments like rest or bracing, to ongoing treatments such as physical therapy, medication, or surgery. This history matters more than you might think.

Part 4

Your Service-Connected Disabilities

If you are already service-connected for any conditions, please bring a complete list — all of them. You may be surprised how an existing connection can open the door to additional compensation through secondary service connection.

Not sure if something is relevant? Bring it anyway. In our experience, the details Veterans overlook are often the ones that make the biggest difference.
